ABSTRACT

Objective:To evaluate the efficacy and safety of gold micrhenedle radiofrequency and other photoelectric methods in the treatment of facial acne depression scar by using a meta-analysis.Methods:From January 2015 to August 2022, gold microneedles and radio frequence for treatment of facial acne depression scar of randomized controlled trial were retrieved from CNKI, Wanfang Database, VIP, China Biomedical Literature Service System, PubMed database, Cochrane Library and Embase database, including 12 papers. There were 6 Chinese and 6 English literatures, with a sample size of 612 cases.Results:Gold microneedling radio-frequency showed better efficacy in the treatment of facial acne depression scar ( P<0.05). After subgroup analysis, the effective rate in the observation group was higher than that in the control group after 4 treatments, and the difference was statistically significant ( P<0.05). Clinical acne scarring assessment scale, pain score and recovery time had statistically significant difference ( P<0.05). Conclusions:Gold microneedling radiofrequency alone or in collaboration with other photoelectricity in the treatment of acne depression scar has short rest period, slight pain, and obvious improvement of scar effect. However, the improvement effect on icicle depression scar is limited.

ABSTRACT

Objective:To investigate the clinical effect of 5-aminolevulinic acid (ALA) photodynamic therapy (PDT) combined with CO 2 fractional laser on hypertrophic scars. Methods:From 2017 to 2019, 20 patients [17 males, 3 females, aged (24.4±4.24) years] with mandibular acne keloid in the Department of Dermatology of Air Force Medical Center, were treated with ALA-PDT combined with CO 2 fractional laser treatment, once a month, 3 times in total, and were followed up for 6 months. The efficacy, safety, and recurrence were evaluated by the Vancouver scar scale (VSS) score. Results:The effective rate was 100% after three treatments. The VSS score decreased significantly after the first treatment, and as the number of treatments increased, the VSS score further decreased. The first improvement after treatment was thickness and hardness. There was no recurrence during the 6-month follow-up. The main adverse reactions were pain, erythema and pigmentation.Conclusions:ALA-PDT combined with CO 2 fractional laser can be used as a new treatment for hypertrophic scars.

ABSTRACT

Objective:To evaluate the clinical efficacy and safety of a cream containing madecassoside, 5% panthenolon skin repairing after nonablative fractional 1 565 nm laser therapy.Methods:A total of 84 patients who received nonablative fractional laser surgery in our hospital from April 2017 to April 2018 were included as research objects. The patients were divided into observation group and control group by random number table method, 42 cases in each group. The control group was treated with routine facial intervention after operation, while the observation group was treated with a cream containing madecassoside, 5% panthenolon skin repairing on the basis of routine intervention. The postoperative skin barrier function of the two groups were recorded and compared through skin property system and VISIA complexion analysis system.Results:At 1 and 2 weeks after operation, the sebum content and cuticle water content in the observation group were higher than those in the control group, and the transdermal water loss in the observation group was less than that in the control group, and the differences were statistically significant ( t=4.927, 7.833, 12.430, 4.538, 10.083, 8.017, P<0.05). The erythema index (EI) and melanin index (MI) of the observation group were lower than those of the control group at 1 and 2 weeks after operation, and the incidence of complications was significantly lower than that of the control group ( t=2.392, 2.807, 3.485, 3.009, P<0.05). Conclusions:The application of a cream containing madecassoside, 5% panthenolon in skin repairing is helpful to enhance the moisturizing effect, reduce the complications and promote the early recovery of patients with skin trauma after fractional laser operation.

ABSTRACT

Objective:To evaluate the efficacy and patients' satisfaction of the intense pulse light (IPL) and M22-Resur Fx non-ablative fractional laser (NAFL) for the treatment of lateral static crow's feet.Methods:A prospective split-face, randomized, controlled study of 60 consecutive patients with lateral static crow's feet was performed. The patients aged 30 to 55 years, who were enrolled in the Department of Ophthalmology, Beijing Tongren Hospital, Capital Medical University from January 2018 to January 2020. The two sides of each patient's face were randomized to receive different treatment, and according to their treatment, the patients were divided into 3 groups with 20 cases each: Group A, in which 20 patients' one side face was treated with IPL, and other side was treated with NAFL; Group B, in which one side face was treated with IPL, and another side was treated with IPL immediately followed by NAFL; Group C, in which one side face was treated with NAFL, and another side was treated with IPL immediately followed by NAFL. The improvements including the water content of stratum corneum of skin and the patients' satisfaction, as well as the improvement of the lateral static crow's feet were evaluated.Results:The IPL plus NAFL combined treatment group and the NAFL treatment group showed the effective rate of the improvement for lateral static crow's feet of patients on days 60 and 90 was statistically significant ( P<0.05) compared with that before the treatment. And the improvement for lateral static crow's feet of patients on days 60 and 90 in combined treatment side was significantly different from either NAFL side ( P<0.05). 60 days and 90 days after treatment, the satisfaction of combined treatment side was significantly higher than that of either NAFL side or IPL side ( P<0.05). Conclusions:The treatment with an optimized IPL immediately followed by a NAFL is clinically superior to those with the NAFL or IPL alone for improving the lateral static crow's feet of patients. The satisfaction treated by NAFL combined with IPL is higher than using the NAFL or IPL alone.

ABSTRACT

Objective:To investigate the effectiveness and safety of soothing moisturizing repair cream on acne depressed scar exfoliative fractional laser wound repair.Methods:From October 2018 to June 2020, the Department of Dermatology, Qingdao Haici Hospital Affiliated to Qingdao University took 33 patients with acne depressed scars as the research object, including 8 males and 25 females, aged from 20 to 36 years (29.6±8.6) years. The left and right face comparison method was adopted. After laser operation, the trial side was given a soothing moisturizing repair cream, and the control side was given a placebo. By collecting the patient's facial pictures and objective skin data before and after the laser operation, 1 h, 1 d, 3 d, 7 d, 21 d, and combined with the researcher's semi-subjective evaluation and patient's subjective evaluation the wound skin reaction and wound healing were observed.Results:At 1, 3, 7, 21 days after laser operation, the skin water content of the test side was higher than that of the control side ( P<0.05), and the skin water loss was lower than the control side ( P<0.05); at 3, 7, 21 days, the skin pigment of the test side was lower than the control side ( P<0.05); at 3, 7 d, the test side skin erythema index was lower than the control side ( P<0.05); at 1, 3, 7 d, the test side wound skin erythema, edema, dryness and tightness, etc. were better than the control side ( P<0.05). The duration of pain, crusting time, scab removal time, and complete healing time of the wound on the test side were shorter than those on the control side ( P<0.05). The patient's satisfaction with the moisturization and comfort of the nursing products on the trial side was better than that on the control side ( P<0.05). Conclusions:There is no adverse reaction to the soothing moisturizing repair cream after laser surgery, which can better inhibit skin inflammation, reduce post-inflammatory pigmentation, promote skin healing, and help repair the wound after laser surgery.

OBJECTIVE@#To observe and compare the clinical efficacy of CO₂ fractional laser in the early control of scar post-secondary repair in patients with a cleft lip.@*METHODS@#In the treatment group, 43 patients with secondary repair of cleft lip were treated via CO₂ fractional laser. The control group covered 70 patients post-cheiloplasty. The effect in the two groups after six months was compared. The duration from the beginning of the laser treatment to surgery and patient gender were analyzed to determine if they affected the efficacy of the laser treatment for scars.@*RESULTS@#1) The curative effect in the treatment group was better than that in the control group (P<0.000 1), the total effectiveness rate was 90.7% in the treatment group. 2) No significant statistical difference in efficacy was observed between men and women (P=0.487). Moreover, no significant statistical difference in efficacy (P=0.055) was observed among patients one year after surgery, within the duration of <3 months, and within the duration of ≥3 months.@*CONCLUSIONS@#CO₂ fractional laser had a definite effect on the treatment of scar post-secondary repair in patients with a cleft lip. No significant correlation was observed between treatment effect and patient gender, and no difference was noted among patients one year after surgery. In the beginning of the laser treatment, no difference was observed between the durations of <3 months and ≥3 months after the reconstruction, Therefore, early intervention of scars a year after secondary repair of a cleft lip can achieve good results.

ABSTRACT

Objective@#To explore the effect and safety of fractional 1565 nm laser on white striae gravidarum (SG) in abdomen and observe the influencing factors of treatment effect.@*Methods@#Thirty-six patients, aged 24-39 years old, were randomly divided into two groups(group A, n=16, group B, n=20): patients in group A underwent two passes of laser treatment: striae were locally scanned at moderate level energy(35 mJ/μb) with high density(300 dot/cm2), and high level energy (45 mJ/μb)with low density(150 dot/cm2)was adopted for the whole area.Group B received one pass of treatment at moderate energy with high density 35 mJ/μb, 300 dot/cm2). All patients received three laser treatments at a 6 weeks interval. Improvement of SG appearance was assessed by blinded reviewers, 3 months after the last treatment, using a percentage category scale. Meanwhile, patients satisfaction was evaluated. The impact of skin aging on laser treatment was evaluated. Patient satisfaction scores and side effects were evaluated.@*Results@#All patients completed the overall trial. The average improvement of scores was 1.69±0.95, which contributed to a fairly good response rate of 88.89%. The patients satisfactory and very satisfactory rate was 58.3%(21/36). There was no significant difference in improvement score between group A(1.88±0.89) and group B(1.55±1.00) (P< 0.01). The operation duration of group B was shorter than group A, (29.6±4.9)min vs.(53.7±5.5)min, P<0.05. There was a moderate negative correlation between aging and laser treatment effect (r=-0.553). The abnormal color of skin(erythema and post-inflammatory hyperpigmentation) in group A lasted longer than group B, (9.17±4.03)d vs. (6.73±2.29)d, P<0.05.The pain score during treatment was tolerable, with the score of 2.61±0.76.@*Conclusions@#The treatment with fractional 1565 nm laser can improve the appearance of SG. It is more timesaving when the entire implicated area treated by one pass at moderate level energy with high density. Skin aging will discount the effect.

ABSTRACT

Objective To explore the application of Lux1540 nm non-stripping array laser in fa-cial rejuvenation .Methods A total of 100 patients were collected for Lux1540 nm non stripped lattice laser treatment in patients with facial skin aging ,once a month ,totally four times treatment ;1 month after the treatment ,the skin of patients was analyzed by skin image analyzer VISIA for quantitative e-valuation .Results After 4 treatments ,the skin wrinkles ,texture ,pores ,skin roughness ,brown spots ,erythrocyte and purple spots were all improved ,with statistically significant differences (P <0 .05) ,while the changes of ultraviolet spots were not obvious ,and the differences were not statistical-ly significant (P > 0 .05) .After treatment ,80 patients were satisfied ,17 were comparatively satisfied , and 3 were dissatisfied ,with a satisfactory rate of 97% .All patients had needle-like pain during the treatment ,which could be tolerated without local anesthesia ointment .Ice compress was given after treatment ,which significantly alleviated the discomfort after treatment .Only 3 cases had mild pig-mentation .During the treatment and follow-up ,no adverse events such as skin redness and swelling , pigmentation and pruritus were found .Conclusions Lux1540 nm non-ablative dot array laser has posi-tive efficacy ,high safety and few side effects in facial rejuvenation ,and it is an effective method to treat facial skin aging .

ABSTRACT

Objective To evaluate the safety and efficacy of the combined treatment with an optimized intense pulsed light (IPL) and a non-ablative fractional laser (NAFL) for facial rejuvenation.Methods A prospective,split-face,randomized,controlled study was conducted.A total of 22 testees with facial photoaging,who aged from 35 to 55 years,were enrolled into this study from the Department of Dermatology of Peking University First Hospital between March and June in 2017.By a random number table,the two sides of each testee's face were randomized to receive combined treatment with optimized IPL immediately followed by non-ablative 1 565 nm Erbium:Glass fractional laser (combined treatment group) or non-ablative 1 565 nm Erbium:Glass fractional laser alone (NAFL group) once every month for 3 sessions.Before the treatment,60 and 90 days after the treatment (1 month after the second and third treatment respectively),photos of the treatment regions were taken,skin physiology parameters (including skin melanin,erythema indices,water content of the stratum corneum,transepidermal water loss [TEWL],skin flexibility and glossiness) were measured,and subjective and objective clinical evaluation was carried out.After each treatment,adverse reactions were assessed by two dermatologists independently,including facial erythema,swelling and crusting,desquamation,pigmentation and pains.Results During the treatment course,1 testee dropped out due to pains,another 1 testee dropped out for personal reasons,and 20 testees completed the treatment and follow-up.The combined treatment group showed significantly decreased melanin indices on days 60 and 90 (152.9 ± 36.9 and 155.0 ± 38.1,respectively) compared with that before the treatment (168.4 ± 41.3,F =5.321,P < 0.05).On day 60,the melanin index was significantly lower in the combined treatment group than in the NAFL group (159.4 ± 35.3,P < 0.05).However,the melanin indices on days 60 and 90 in the NAFL group (159.4 ± 35.3,156.7 ± 36.3) did not significantly differ from that before the treatment (165.9 ± 35.4,P > 0.05).No significant difference was observed between the pre-and post-treatment erythema indices in either of the two groups (both P > 0.05).The water content of the stratum corneum on days 60 and 90 significantly increased compared with that before the treatment in both the combined treatment group (F =21.795,P < 0.001) and NAFL group (F =21.798,P < 0.001),while the TEWL on days 60 and 90 significantly decreased compared with that before the treatment in both the combined treatment group (F =8.848,P =0.001) and NAFL group (F =5.833,P < 0.05).However,there were no significant differences in either of the water content of the stratum corneum or TEWL on days 60 and 90 between the two groups (P > 0.05).On days 60 and 90,the combined treatment group and NAFL group both showed significantly increased skin flexibility (P< 0.05,0.001,respectively) and glossiness (both P < 0.001) compared with those before the treatment.On day 90,the skin glossiness in the combined treatment group was higher than that in the NAFL group (P < 0.05).The short-term adverse reactions included transient pain,erythema and swelling which lasted 2-3 days,and slight desquamation.The main adverse reaction was mild local pigmentation,which lasted 2-3 months and then subsided gradually.Conclusion The 3 sessions of treatment with an optimized IPL immediately followed by a 1 565 nm NAFL is clinically superior to those with the NAFL alone for improving the facial pigmentation and skin glossiness,and the adverse reactions are usually transient and mild.

ABSTRACT

Objective To preliminarily optimize the energy and density of laser radiation for the early treatment of hyperplastic scars in a rabbit ear model,and to explore possible therapeutic mechanisms.Methods Sixty-one hyperplastic scars were successfully established on the ears of 10 healthy New Zealand white rabbits with large ears,and randomly divided into 2 groups:1-week group (30 scars) and 3-week group (31 scars).These 2 groups were separately divided into 5 subgroups:group A treated with laser at a density of 100 PPA and an energy of 10 mJ,group B with laser at a density of 100 PPA and an energy of 50 mJ,group C with laser at a density of 169 PPA and an energy of 10 mJ,group D with laser at a density of 169 PPA and an energy of 50 mJ,and group E receiving no treatment.There were 6 scars in each group,except the group E in the 3-week group.Two healthy New Zealand white rabbits with large ears were not subjected to modeling,and served as group F (blank control group).Immunohistochemical study was performed to determine the expression of matrix metalloproteinase (MMP)-13 in the skin tissues from the rabbit ears 1 week after the treatment.Three weeks after the treatment,the skin tissues from the rabbit ears were subjected to hematoxylin-eosin (HE) staining and Masson staining.Then,the structure of scars was observed,and scar elevation index was calculated.Statistical analysis was carried out by Kruskal-Wallis H test for the comparison of scar elevation index,and by one-way analysis of variance (ANOVA) for the comparison of the average absorbance value of MMP-13.Results As HE staining revealed,the groups A,B,C and D all showed thicker dermis and increased number of collagen fibers compared with the group F (normal skin tissues),but showed thinner dermis,decreased number and more ordered arrangement of collagen fibers compared with the group E (untreated scar tissues).No obvious difference was observed in the thickness of the dermis among the groups A,B,C and D.The scar elevation index significantly differed among the 6 groups (H =22.757,P < 0.05).Multiple comparisons showed that the scar elevation index was significantly lower in the groups B,C and D (2.597 ± 0.344,2.850 ± 0.282,2.658 ± 0.134,respectively)than in the group E (3.460 ± 0.583,all P < 0.05).As Masson staining revealed,the groups A,B,C and D all showed thinner dermis and more irregular arrangement of collagen fibers compared with the group E.However,no obvious differences were observed in the dermal thickness or number of collagen fibers among the groups A,B,C and D.Immunohistochemical study showed that the expression of MMP-13 was significantly higher in the high-energy (50 mJ) laser groups than in the low-energy (10 mJ) laser groups (P < 0.05) at the same laser density.With the same laser energy,the expression of MMP-13 was significantly higher in the group A than in the group C (P < 0.01),but there was no significant difference between the group B and D (P > 0.05).Conclusions Non-ablative fractional laser is effective for the treatment of early-stage hyperplastic scars.At the same laser density,50-mJ laser was superior to 10-mJ laser for the treatment of hyperplastic scars,likely because high-energy laser can stimulate the recombination of extracellular matrices and up-regulated MMP-13 expression to a greater extent.

ABSTRACT

Objective To assess the efficacy and safety of nonablative 1 565-nm Er:Glass fractional laser in the treatment of striae gravidarum.Methods The left and right sides of the abdomens of 30 subjects with striae gravidarum were divided into 2 groups by simple randomization:treatment group treated with nonablative 1 565-nm Er:Glass fractional laser every 4 weeks for 3 sessions,and control group receiving no treatment.These subjects were followed up at the baseline,4th,8th and 12th week.At the same time,photos were taken,the skin flexibility was evaluated,and the width of striae was measured.Four weeks after the final treatment,clinical improvement was evaluated by 2 doctors,who were blind to the therapeutic protocol,according to clinical photos and three-dimensional (3D) images,and satisfaction scores were also evaluated.Skin biopsies were taken from the left and right sides of the abdomens of 3 subjects,and subjected to histopathological examination.Four months after the final treatment,all the subjects were telephoned for evaluation of adverse reactions and recurrence.Results A total of 27 subjects completed the whole trial.Four weeks after the final treatment,25 (92.6%) of the 27 subjects were considered to achieve an improvement by doctors.However,21 (77.8%) of the 27 subjects considered themselves to achieve an improvement.After 3 sessions of treatment,the treatment group showed a significant decrease in the largest width of striae from 4.852 mm to 3.296 mm (P < 0.001) and a significant increase in skin flexibility from 0.803 to 0.878 (P < 0.001).Histopathologically,the control group showed obviously atrophic epidermis and decreased collagen and elastin in the dermis,while the treatment group showed thickened epidermis and dermis,extended rete ridges,and increased collagen and elastic fibers which arranged more regularly.Immediate adverse reactions during the treatment included erythema and edema,and other adverse reactions included mild crust,itching and post-inflammatory hyperpigmentation (PIH).PIH was improved at 4 months after the final treatment compared with that at 4 weeks after the final treatment.Conclusion The nonablative 1 565-nm Er:Glass fractional laser can markedly improve the width and appearance of striae gravidarum,and increase skin flexibility in the treatment region.

ABSTRACT

Objective To study the therapeutic effect of ultra pulsed carbon dioxide fractional laser on treating hypertrophic scar after burn.Methods Forty-five patients with hypertrophic scar after burn were selected.Each laser treatment included an ultra-pulse mode and a Scaar FX mode scan irradiation,and the treatment wasconducted once a month for 4 to 6 times.In the ultra-pulse irradiation,the energy was 150~175 mJ,frequency was40 Hz,hole spacing was about 4~5 mm,and irradiation time per hole was 2~3 s,and in Scaar FX mode energy was60~150 mJ,frequency 250 Hz,and density 1% to 3%.Before the first treatment and 6 months after the end of thetreatment,the Vancouver scar scale (VSS) was used to score the scar morphology,and the visual analog scale (VAS)was used to score the scar pain and itching.The effect of the treatment was classified as significant,effective andinvalid.In addition,adverse reactions occurred during the treatment were recorded.Results After six months of thetreatments,the VSS score,VAS pain score,and VAS pruritus score of the patients were 4.16±1.72,1.58±0.62,and1.24±0.74,respectively,while the pre-treatment values were 10.17±1.96,2.98±0.89,and 2.31±0.97,respectively.The differences were statistically significant (all P<0.01).The treatment was effective in all patients,and the effectwas significant in 35 cases,effective in 10 cases,and no recurrence occurred within 6 months after treatment.In 3patients,blisters appeared on the scar after the first treatment,and the blister collapsed to form a wound.After thedressing change,the blisters healed after 7 to 10 days.Conclusion Ultra pulsed carbon dioxide fractional laser iseffective in treating hypertrophic scar after burn,with mild adverse reactions and no relapse,which is worthy ofclinical promotion.

ABSTRACT

Objective: To compare the curative effects and adverse reactions of fractional carbondioxide laser combined with tacrolimus ointment and tacrolimus ointment alone in the treatment of localized vitiligo, and to evaluate their safeties. Methods: The clinical materials of 98 patients with localized vitiligo were retrospectively analyzed. The patients were divided into two groups according to their wishes. The patients in control group (47 cases) were treated with tacrolimus ointment every morning and evening only, and the patients in observation group (51 cases) were treated with fractional carbondioxide laser combined with tacrolimus ointment and the laser was used once every 4 weeks. The treatment lasted for 12 weeks. Compound Glycyrrhizin Tablets were used according to the conditions (3 pieces each time, 3 times a day, a total of 12 weeks). The follow-up period was once every 2 weeks. The clinical efficacy and adverse reactions of the patients in two groups were assessed in the 12th weeks after treatment. Results: The onset time of treatment of the patients in observation group was shorter than that in control group (t=13. 82, P=0. 001), and the total effective rate of the patients in observation group was higher than that in control group (χ2 = 4. 72, P<0. 05). The effective rates of the faciocervical lesion of the patients in observation group and control group were higher than those of the trunk lesion (χ2 = 23. 78, P<0. 05; % = 12. 79, P<0. 05). The effective rate of the faciocervical lesion of the patients in observation group was higher than that in control group (χ2= 5.75, P< 0. 05); there were no serious adverse reactions in all patients. Conclusion: The combination of fractional carbondioxide laser and tacrolimus ointment in the treatment of localized vitiligo has the advantages of quick effect, good efficacy and high safety, and is worthy of clinical application.

ABSTRACT

Objective To investigate the efficacy and side effects of carbon dioxide (CO2) fractional laser for the treatment of suborbital wrinkles.Methods Twenty-one female patients with suborbital wrinkles received a single treatment of CO2 fractional laser at fluences of 20-30 mJ with density of 10 %-15 %.Patients were photographed standardly and evaluated at baseline,one and six months after the treatment.Wrinkle improvements were graded in 5 levels:1 (no improvement or worse),2 (1% 25% improvement),3 (26%-50% improvement),4 (51%-75% improvement),and 5 (76%-100% improvement).Patients rated the pain intensity by a visual analog scale (VAS,0 10).The side effects,such as hyperpigmentation,were observed and recorded.Results After one month follow-up,3-level improvement was observed in 7 patients (33.3%),2-level in 9 patients (42.9%);and after six months,3-level improvement was observed in 9 patients (42.9%),2-level in 8 patients (38.1%),but there was no statistically significant difference between one month and six months (P >0.05).The overall patient satisfactory rate was 81.0%.The average pain of VAS was 4.24± 1.73,7 patients (33.3%) had hyperpigmentation which faded within three months,and no patients had scars.Conclusions Single CO2 fractional laser treatment can improve the suborbital wrinkles in a cerrain extent with few side effects.

ABSTRACT

Objective To investigate the effect of fractional laser on the expression of stem cell factor (SCF) and c-kit receptor in body surface of guinea pig.Methods 20 guinea pigs were adopted.The back skin of each guinea pig was divided into four zones:A zone (the skin was not treated),B zone (the skin was treated by fractional laser at a low fluence:20 mJ/cm2),C zone (the skin was treated at a medium fluence:60 mJ/cm2),and D zone (the skin was treated at a high fluence:120 mJ/cm2).The immunohistochemical method was used to detect the expression of SCF and c-kit in body surface of guinea pig.The results were quantified using an HPIAS-1000 system.Results One week later,the SCF expression level in B,C and D groups was increased obviously,which showed statistical difference compared with A group (P<0.05);the c-kit expression level in C and D groups was increased obviously,compared with A group (P<0.05).4 weeks later,the SCF expression level in C and D groups was higher than A and B groups;furthermore the level in D group was higher than C group (P<0.05);the c-kit expression level in D group was higher than A and B groups (P<0.05).Conclusions The fractional laser could generate the curative effect by increasing the expression of SCF and c-kit in body surface.

ABSTRACT

Objective To evaluate the clinical effects and safety of Er:YAG fractional laser for facial rejuvenation.Methods Altogether 559 patients aged from 32 to 58 years old with obvious pigmentary lesions and wrinkles in the face were given 2 courses of restoration treatment with the Pixel Er:YAG fractional laser.The patients paid a return visit a month later after the last treatment.With the results of clinical observation and photograph-based comparison,the therapeutic effects were graded by disappearance of pigmentary lesions and elimination of facial wrinkles.Complications such as hyperpigmentation,depigmentation and hyperplastic scar were observed.Results Among the 559 patients, there were 103 cases (18.4%) of slight effect, 378 cases (67.6%) of moderate effect, and 78 cases (14%) of excellent effect,with the total effective rate of 100% and a satisfactory rate of 81.6%.The was no complication such as hyperpigmentation,depigmentation and hyperplastic scar occurred.Effective maintaining rate within 6 months was 67%.Conclusion Er:YAG fractional laser is safe and effective for facial rejuvenation and worthy of promotion.

ABSTRACT

Objective To explore the effects of wound healing and histological changes by utilizing ultrapulse CO2 fractional laser for the treatment of superficial scar in dermal-epidermal junction (DEJ) in rabbit ears.Methods We adopted traditional iodophor treatment and moist exposed burn ointment treatment repairing the wound and utilizing ultra pulse CO2 fractional laser for the treatment of rabbit ears superficial scar.The superficial scars in rabbit ear were induced with ultra pulse CO2 fractional laser.In 1,2,4,8 weeks tissue samples would be taken.Using electron microscopy we observed DEJ ultrastructure and light microscopy to observe the histological changes of the dermal papilla and PAS staining for epidermis basement membrane.Results The wound healing time of moist exposed burn ointment group was shorter than that of iodophor group (P<0.05).The number of hemidesmosomes and anchoring fibers in the moist exposed burn ointment group on the 8th week was more than the 1th week,The number of hemidesmosomes and anchoring fibers in the iodophor group on 8th week was more than 1th week (P <0.05).The number of hemidesmosomes and anchoring fibers in the iodophor group was less than the moist exposed burn ointment group (P<0.05).Conclusions U1trapulse CO2 fractional laser can cause ultra structural changes in scarring dermal-epidermal junction area,wet wound healing environment promotes the reconstruction of DEJ district organization,and rebuilding the function of the scar skin has the positive significance.

Las cicatrices de acné, especialmente las de tipo atrófico, constituyen una complicación frecuente del acné vulgar que se asocia a un importante impacto psicológico. Múltiples terapias se han utilizado para mejorar su aspecto, sin embargo, ninguna ha demostrado ser completamente eficaz y segura. El tratamiento quirúrgico en acné cicatricial incluye técnicas como escisión elíptica, escisión o elevación por punch, subcisión y distintos tipos de láser. El objetivo de esta revisión es presentar la evidencia disponible respecto a las técnicas quirúrgicas y dispositivos láser utilizados en cicatrices atróficas de acné.


Acne scars, especially the atrophic type, are a common complication of acne vulgaris, which is associated with a major psychological impact. Multiple therapies have been used to improve its appearance, however, none has proven completely effective and safe. Surgical treatment includes surgical techniques such as elliptical excision, punch excision or elevation, subcision and different types of lasers. The aim of this review is to present the available evidence regarding the surgical techniques and laser devices used in atrophic acne scars.

ABSTRACT

Laser treatment for scars has improved over the past three decades. Autologous platelet-rich plasma (PRP) derived from whole blood is immunologically inert and contains a proper ratio of growth factors and cytokines. Here we describe the case of a 29-year-old female patient with a hypertrophic scar on her right shoulder caused by an operation performed in 2012. The patient underwent 11 laser therapy sessions with a fractional carbon dioxide (CO2) ablative laser system (LineXel) and two PRP injections. Her scar was evaluated with the Vancouver Scar Scale (VSS), and the baseline and post-treatment scores were 11 and 3, respectively. After treatment, the dimensions and volume of the scar were diminished, and contour, texture, and pigmentation had also improved compared to baseline. The patient reported less pain, swelling, and pigmentation following PRP combination ablative laser therapy. This case provides further evidence of the potential benefits of PRP as an adjuvant to fractional laser in reducing hypertrophic scars.
